IRMAA for 2027: How to Request a Review

Understand Medicare income surcharges, read your notice, and prepare an SSA review or life-changing-event request without assuming future rates.

Quick questions and answers

What is IRMAA?

IRMAA is an income-based addition to Medicare Part B and drug coverage premiums.

What income counts for IRMAA?

Social Security uses adjusted gross income plus tax-exempt interest, called MAGI.

Can lower income change IRMAA?

A qualifying life-changing event and documented income reduction can support a request for a new Social Security determination.

Start with your notice, not a guessed rate

IRMAA means Income-Related Monthly Adjustment Amount. It is an extra Medicare charge tied to income. It can affect Part B and Part D. If you receive a notice for 2027, read the income year, filing status, and instructions used in your own decision.

This guide does not list 2027 brackets, premiums, or surcharge amounts. The local calculator contains 2026 figures. A future-year notice should not be checked against an older table as if the amounts were unchanged.

Use the IRMAA calculator with 2026 figures to learn how income and filing status fit into the calculation. Use Social Security's current notice and official guidance for an actual determination. The calculator does not replace that decision.

Understand the income information

The local calculator explains that modified adjusted gross income, or MAGI, generally adds tax-exempt interest to adjusted gross income. Find those items on the relevant tax records. Do not use take-home pay, bank deposits, or a guessed yearly salary in their place.

For its 2026 calculation, the source tool uses the 2024 tax return, or 2023 if that return is unavailable. Those years describe the existing tool. They are not instructions for which return Social Security must use for your 2027 notice.

Compare the filing status and income shown on your notice with your records. If the figures or tax return information seem wrong, ask Social Security how to correct or challenge the decision. Keep the notice intact so you can follow its instructions and deadlines.

A new determination and an appeal are different

A request based on a qualifying life-changing event that lowers income is different from a formal appeal of an incorrect decision. The official SSA-44 form is used to request a new IRMAA determination after such an event. It is not a general promise that every income drop qualifies.

If you think Social Security made an error, follow the appeal instructions on the notice. If you filed an amended tax return, the local checklist says that uses a separate Social Security contact route. Ask which process fits your facts before filling out forms.

Use the SSA-44 document checklist to organize an event-based request. The checklist is a folder index, not the official form. It cannot submit a request, decide eligibility, or guarantee that your charge will fall.

Check the reason your income changed

The checklist includes work stoppage or retirement, work reduction, marriage, divorce or annulment, death of a spouse, loss of income-producing property, loss of pension income, and an employer settlement payment. Select the event that matches your records and confirm its requirements with Social Security.

Describe the actual event, when it happened, and how it affected income. Avoid choosing a category just because it sounds close. Social Security decides whether the event and evidence meet the rules.

If your income fell for another reason, ask what options may apply. Do not assume that the checklist covers every possible review route. The right answer depends on the notice, the income record, and the reason for the change.

Build a useful document folder

Keep the IRMAA notice, relevant tax records, event evidence, and income estimates together. The local checklist says a filed return may be supported by a signed copy or an IRS transcript. If you use an estimate, provide the signed return after filing as directed.

Prepare adjusted gross income and tax-exempt interest for the affected year. Include a next-year estimate when the official form asks for it and your circumstances call for it. Label estimates clearly so they are not mistaken for filed returns.

  • Read the determination notice and identify the issue.
  • Confirm whether you need a new determination, an appeal, or another route.
  • Use the current official form when a form is required.
  • Gather evidence of the event and its effect on income.
  • Ask which originals or certified copies are acceptable.
  • Keep copies and a submission or appointment record.

Use the official form for personal identifiers and signatures. Keep those details out of the planning tool. A saved worksheet can contain private notes, so store it carefully and share only what the official process needs.

Ask focused questions and track the response

When you contact Social Security, have the notice and your timeline ready. Ask what information is missing, which request route applies, and how to submit the evidence. Follow the current instructions from Social Security rather than a remembered mailing address or an old form.

Do not treat a submitted request as an approved reduction. Keep any acknowledgment and later decision. Read the response to see whether Social Security changed the determination or asked for more information. Ask about billing while the request is being reviewed.

If you still disagree, read the new notice's review instructions. This guide gives no fixed appeal deadline because the notice is the place to find the deadline that applies. Make the date visible in your own calendar and keep proof of timely action.

Keep the surcharge separate in your budget

Part D IRMAA is separate from your drug plan premium. The calculator's results are per person. A household estimate should therefore not assume that a displayed amount covers everyone.

The annual-cost comparison worksheet can help organize premiums and expected spending. Use figures for the same coverage year. Do not mix older calculator amounts with future plan documents without clearly labeling the difference.

After Social Security responds, update your budget from the official decision. Return to the free document checklist to keep your evidence organized. Clear records help you explain the request; they do not establish a right to a reduction.

Frequently asked questions

Does this guide provide 2027 IRMAA brackets?

No. The linked calculator uses 2026 figures. Use your actual notice and current Social Security guidance for the coverage year being reviewed.

Is SSA-44 the same as a formal appeal?

No. It requests a new determination after a qualifying life-changing event that lowers income. Follow your notice for a formal appeal of an incorrect decision.

Does retirement automatically remove IRMAA?

No. Work stoppage or retirement is a category in the local checklist, but Social Security decides whether your event, income, and evidence support a new determination.
