SSA-44 is a request for a new IRMAA determination following a qualifying life-changing event that lowers income. It is different from a formal appeal of an incorrect determination. An amended tax return uses a separate SSA contact route; follow the instructions and deadlines on your notice if you disagree with a decision.
Entries stay in this page’s memory; these tools do not store or send them. Refreshing or clearing removes them. Save a PDF to retain a copy. Do not enter Medicare numbers, Social Security numbers or upload documents.

Use your browser’s Print dialog and choose Save as PDF. Large-print output includes your current entries, results, assumptions and sources. Blank fields remain blank; enter 0 only when a cost is truly zero.
Income evidence and submission
If a relevant return has been filed, SSA-44 asks for a signed copy or IRS transcript. If you use an estimate, provide the signed return after filing. Prepare AGI and tax-exempt interest for the affected year and any next-year estimate. Use the official form for personal identifiers and signatures; this checklist cannot submit a request.
Follow SSA’s current online, mail or appointment instructions linked below. SSA decides which evidence is acceptable and whether IRMAA changes; no approval or savings are promised. This folder index helps organize the request and does not establish eligibility.
